Pada pukul 09:30, di desa [[Peraliya]], dekat [[Telwatta]], gelombang tsunami menerjang kawasan pantai. Kereta berhenti ketika air meluap mengelilingi kereta. Warga lain berdiri di belakang kereta supaya terlindungi dari gelombang. Gelombang pertama membanjiri gerbong dan membuat para penumpang panik. Sepuluh menit kemudian, gelombang besar menyapu dan menabrakkan kereta api dengan pepohonan dan rumah di pinggir rel. Orang-orang yang berlindung di belakang kereta tewas seketika.
 
Delapan gerbong sangat penuh dengan penumpang sampai-sampai pintunya tidak bisa dibuka ketika terendam air. Hampir semua orang di dalamnya tenggelam karena gerbong digulung air beberapa kali. Penumpang di atas kereta jatuh ketika gerbong dibalikkan oleh gelombang. Sebagian besar korban tewas akibat tenggelam atau terkena reruntuhan. Lokomotif #591 ''Manitoba'' tersapu sejauh 100 meter (328 kaki) dan mendarat di rawa. Masinis Janaka Fernando dan [[asisten masinis]] Sivaloganathan meninggal dalam tugas.
