Di bangku perkuliahan, Rorschach diajar oleh psikiater-psikiater ternama seperti [[Eugen Bleuler]]. Pada saat itu, psikoanalisis sedang berkembang, yang membuat Rorschach teringat kembali pada gambar bercak tinta masa kecilnya. Rorschach penasaran kenapa orang-orang sering kali melihat hal-hal yang berbeda pada bercak tinta yang sama. Terdorong oleh hal ini, Rorschach mulai memperlihatkan bercak tintanya pada anak-anak sekolah dan menganalisis respon mereka, dan mulai mengembangkan tes psikologi berbasis bercak tinta.
Pada bulan Juli 1914, Rorschach kembali ke Swiss, bekerja sebagai Asisten Direktur di rumah sakit jiwa [[Herisau]],<ref name = mhhe/> dan pada tahun 1921 ia menulis buku berjudul ''[[Psychodiagnostik]]'', yang menjadi dasar dari tes bercak tinta.
