Pesawat Helios yang jatuh yang bernomor registrasi 5B-DBY tersebut dilaporkan pernah bermasalah dengan pintu belakang (terbuka saat penerbangan). Untuk mencegah itu terjadi lagi, teknisi Helios mengadakan tes tekanan udara untuk mencari titik kebocoran. Ketika tes itu dilaksanakan, salah satu prosedurnya adalah mengatur tombol tekanan kabin ke posisi "MAN" (manual). Setelah diperiksa, tidak ada tanda-tanda kebocoran. Ketika meninggalkan pesawat (tes dilangsungkan pada malam hari di [[Bandara Larnaca]]), para teknisi itu lupa mengembalikan tombol tekanan kabin ke posisi "AUTO" (otomatis). Inilah yang menyebabkan hilangnya tekanan udara dalam penerbangan tersebut secara perlahan-lahan dan menyebabkan pesawat itu jatuh, menewaskan seluruh penumpang dan awaknya.
