Kereta api Sancaka beroperasi sejak 21 Mei 1997 dan melayani koridor Yogyakarta Tugu - Surabaya Gubeng. Perjalanan sejauh 311 km ditempuh dalam waktu kurang dari 5 jam. Nama Sancaka diambil dari nama seekor ular naga sakti yang siap melindungi.

Kereta api Sancaka berhenti di Stasiun Mojokerto, Stasiun Jombang, Stasiun Kertosono, Stasiun Nganjuk, Stasiun Madiun, Stasiun Solo Balapan, dan Stasiun Klaten.

Kereta api Sancaka(KA 82/83) umumnya membawa lokomotif CC201 dengan stanformasi CC 201-B-2 K2-2 K3 AC-MP2-4 K1-P1, sedangkan KA 81/84 umumnya membawa CC201-aling aling-2K2-M1-5K1 (semuanya retrofit dari Turangga). Bahkan sekarang M1 dari KA81/84 sudah dibatik menjadi bermotif Batik Madura.
