McCarthy mendapat pujian luas atas penulisan naskah dan penyutradaraan film independen The Station Agent (2003), The Visitor (2007), Win Win (2011), dan Spotlight (2015). Untuk Spotlight, ia mendapatkan nominasi Academy Award for Best Original Screenplay dan Academy Award for Best Director.
